Today I saw a 42-year-old female patient who is trying to get pregnant. She is overweight and her insulin level is in the 12.9 range. For individuals undergoing in vitro fertilization (IVF), maintaining optimal insulin levels is important for overall metabolic health and reproductive outcomes. While the exact "ideal" insulin levels may vary depending on individual health status and protocols, general guidelines can be derived from research and clinical practice:
Fasting insulin levels:
As a general rule, fasting insulin levels should be below 25 µIU/ml, with the optimal range being 2–10 µIU/ml. Insulin resistance (elevated levels) can affect ovarian function and fertility, especially in people with conditions such as polycystic ovary syndrome (PCOS). High insulin levels can also affect the environment in the uterus, reducing the chances of implantation.
HOMA-IR (Homeostatic Model Assessment of Insulin Resistance):
The HOMA-IR score, a calculation based on fasting glucose and insulin levels, should ideally be below 2.5. This indicates a healthy insulin sensitivity.
Glucose to insulin ratio:
A ratio above 4.5–5.0 is generally considered an indicator of a good metabolic situation in women undergoing IVF.
Tips for optimizing insulin levels before IVF:
Make sure to eat a balanced, low-glycemic diet rich in whole grains, lean proteins, healthy fats, and vegetables. Minimize refined carbohydrates and sugar.
Regular moderate exercise improves insulin sensitivity and reduces resistance. Find some activity you enjoy and keep it regular.
Medications such as metformin may be prescribed by your doctor to improve insulin sensitivity.
Maintain a healthy weight and find an activity that helps you to manage stress levels, as cortisol influences blood sugar and insulin levels.
